NOTE: This must be done on a desktop or laptop computer, not a mobile device.
1. Go to Audible.com or audible.co.uk
2. Log into your Audible account using your Amazon credentials
3. Click the dropdown link that says “Hi, <your name>”
At the dropdown, click “Listener Page”.
On the resulting page, select the entire URL in your browser bar. This is your review profile link. Ctrl+c to copy it on a PC or Cmd+C on a Mac. Save it in a note pad document or somewhere safe.
In the form for the book you are requesting, paste the link to your review profile. Ctrl+V to paste on a PC or Cmd+V on a Mac.
This is what it should look like. Fill out the rest of the form and you're done!
